I've also copied the /includes/templates/ files in my custom template directory. What else do I have to do to get the blog add-on up and running?

If you pick up custom name for this, durig LC installation, you need to copy the contents of the "admin" folder there. Just tested this module on fresh LC 2.2.8 installation and seems to be ok.
