US State Shipping Module

The sm_state_based_shipping module borrows heavily from the sm_country_based_shipping module that the LiteCart team published.

The rate maps and tables are used in the same way as the country module, except, each state is listed in the map.

The enables different shipping rates for each state, or group of states.
