Hello, sorry I also cannot get it work.
Here the details (version 2.2.2):

country_code,zone_code,min_subtotal
US,AL,30

(United States, Alabama,30 euro minimum)

I have only one currency: value 1.
And also what to do in country without zone?
can eventually use this syntax:

country_code,min_subtotal
US,30

I also installed the addon that remove the zones (optional_province.zip).
In reality what I need is IT,30 (Italy delivery free over 30 euro -net before tax-) and few other countries that I will add when I will understand how does it work.

For all zones in a country just leave out the zone

Order 32 euro net zone IT charge the IT fee.

What a oversight!
The cart was showing the fee but down in the total then was also discounted!
