Shipping Insurance

Does your shipper charge an insurance fee?  UPS and FedEx do.  This mod allows you to add the shipping insurance charge to each order.

Fees can be entered as either percentages or exact values.  You can use a single fee for all orders, or a floating schedule based on the order sub-total.
